​Note Taking

We are committed to providing you with the very best care. So that we can give you more of our time and focus, we utilise a note taking tool called Heidi to assist in capturing the details of your consultation and creating a medical record of your visit. The security of your data and your privacy are important to us. Heidi stores data locally in Australia and is compliant with the Health Records Act 2001 (Vic), the Privacy and Data Protection Act 2014 (Vic) and the Australian Privacy Act 1988.​​

​Accounts

We expect accounts to be settled on the day. In the event of outstanding accounts, the debtor(s) shall pay for all costs incurred by Evinci Pty Ltd in the recovery of any monies owed under this Agreement. You agree to be liable for and indemnify Evinci Pty Ltd. These costs include recovery agent costs, repossession costs, location search costs and solicitor costs on a solicitor/client basis, debt collection commission, and legal fees on an indemnity basis.

Gym Memberships

By purhcase of a gym memberships with Evinci Pty Ltd you agree to abide by the following terms and conditions: ​

• Memberships are provided by Evinci Pty Ltd, but access is provided within the facilities of Thrive Fit. You agree to abide by all rules, codes of conduct and conditions of entry set by Thrive Fit, and understand that access may be revoked by either party for any breach or non-compliance. 

• You voluntarily assume all risks associated with physical activity and exercise. To the fullest extent permitted by law, you release, indemnify, and hold harmless both Evinci Pty Ltd and Thrive Fit (including their respective owners, staff and affiliates) from any liability regarding personal injury, death or property loss/theft occurring on the premises. 

• You confirm that you have voluntarily disclosed any medical conditions that may impact on your ability to exercise and you are medically fit to participate in the gym program as prescribed by Evinci or otherwise independently. You agree to notify a member of the Evinci team if there is a change to your ability to participate due to feeling unwell, new symptoms arise or any other health changes such as newly diagnosed medical conditions.

• You agree that you are liable for any damage to equipment or property of Evinci or Thrive Fit due to negligence or misuse.
